LABELING INSTRUCTIONS FOR BIOPHARMACEUTICAL SHIPMENTS

Special attention to labeling should be exercised with every shipment to avoid any possibility of a shipment being refused by an airline. QuickSTAT will maintain labeling inventory in all of our domestic and international offices.

Types of labels required for Biopharm shipping:

* Infectious Substance Label (if shipment is declared infectious)
* Diagnostic Specimens or Biological Specimens Packed in Compliance with IATA Packing Instruction (if non infectious)
* Dry Ice (Misc. 9) Label (only if shipment is packed on dry ice)
* Proper Shipping Name (typed label or written on the box)
* UN or ID # (typed label or written on the box)
* Name and address of both Shipper and Consignee (try to include individual name and telephone numbers). Indicate the "Responsible Person" for the shipment. If it is the individual shipper or consignee then indicate that this is the case. If it is neither, you must type or write the Responsible Person and telephone number on the box as well.
* Orientation Labels (this side up)
* Please refrigerate, Keep Frozen (if applicable)
* Inner packages comply with prescribed specifications (when using an overpack)

LABELING PREPARATION:

* If you will be using an "Overpack" to send your infectious specimens, you must remember to label both the inner and outer boxes correctly.
* All labeling must be visible, legible and not covered in any way.
